An entrepreneur willing to bet nearly his entire fortune on a single medicine. That is the story Forbes told about Clemens Fischer, a German doctor and entrepreneur who invested more than US$ 250 million in developing a cannabis-based painkiller — and who has just taken a decisive step toward the market.
A billionaire who bet his own fortune
Fischer, 50, built an estimated US$ 1 billion fortune by creating and selling over-the-counter drug and supplement companies. Through the Munich-based Futrue group, he runs around 20 pharmaceutical and research companies. In 2017 he founded Vertanical with his partner Madlena Hohlefelder, with an ambitious goal: to create a "blockbuster" — the industry term for a hugely commercial medicine — this time based on cannabis.
VER-01: the painkiller that aims to replace opioids
The medicine, developed as VER-01, is a standardized extract from a proprietary Cannabis sativa strain, with low levels of cannabinoids (including THC) — enough to relieve chronic pain but not to cause significant psychoactive effects. Its stated goal is to become the first cannabis-based painkiller prescribed by doctors and covered by health insurance, rather than a dispensary product. In clinical trials with around 800 chronic lower-back-pain patients, the company says it observed efficacy comparable to or better than opioids, with fewer side effects and no signs of addiction.
In 2026, the project took its biggest leap: Germany and Austria granted market authorization for the medicine, now branded Exilby®, with a launch planned for September 2026. In the U.S., Vertanical secured FDA "Breakthrough Therapy" designation and is running a Phase 3 study. The backdrop is the opioid crisis: in 2023 alone there were around 125 million opioid prescriptions in the U.S., and more than 80,000 overdose deaths in the country last year.
According to surveys cited by the report, around 90% of Americans support legal access to medical cannabis — a sign of growing acceptance when the substance enters the formal healthcare system, prescribed as a medicine.
